The role involves accepting Hospice, Home Health, and Home Infusion referrals across various settings such as acute care, physician clinics, nursing homes, and rehabilitation centers. The RN maintains an active presence, delivers excellent customer service to discharge planners, social workers, physicians, and patients, and facilitates timely discharges to ensure continuity of care.
Key responsibilities include:
Assessing patient needs and determining eligibility for hospice and home services
Communicating services effectively to healthcare providers and communities
Providing nursing assessments, documentation, and support within regulatory guidelines
Supporting system initiatives for seamless patient care transitions
Qualifications and skills:
Registered Nurse with current licensure, bachelor’s degree preferred
Minimum three years of nursing experience, hospice/home health experience preferred
Knowledge of hospice and palliative care, with certifications such as CHPN a plus
Excellent assessment, communication, and documentation skills
