Role Overview:
Responsible for accepting referrals for Hospice, Home Health, and Home Infusion in various settings such as acute care, clinics, nursing homes, and rehabilitation centers.
Maintains a presence in these environments, providing excellent customer service to discharge planners, social workers, physicians, and patients.
Supports timely discharges and promotes continuity of care within the healthcare system.
Key Responsibilities:
– Facilitate patient intake and referrals
– Communicate effectively with healthcare providers and families
– Ensure compliance with clinical and regulatory standards
– Document assessments and services accurately
– Support system initiatives for seamless patient transitions
Qualifications & Skills:
– Registered Nurse license in applicable state
– Minimum three years nursing experience; hospice and home health preferred
– Strong assessment, communication, and documentation skills
– Self-directed, flexible, and team-oriented
– Certification in Hospice and Palliative Nursing (preferred)
– Knowledge of current clinical guidelines and regulatory requirements
