The Clinical Care Coordinator II provides age-specific patient care and assists in managing patient visits under direct provider supervision in a physician office setting.
Work involves daily patient appointments at the internal medicine office located in Waldemere Medical Plaza. Requires BLS/CPR certification within 30 days of hire and graduation from an accredited nursing or diploma program. Preferred qualifications include at least one year of outpatient clinical experience, familiarity with electronic health records and practice management software, and proficiency in Microsoft Office applications. Must hold an active FL Licensed Practical Nurse (LPN) license. Work schedule is Monday to Friday with no weekends or holidays, and background screening is required for all care providers working with vulnerable populations.
