Physician

We are hiring a full-time Physician passionate about team collaboration, community-based care, and supporting the independence of elderly and disabled adults.

Functions as part of an interdisciplinary team, developing care plans, monitoring health, and providing geriatric and medical expertise.

Supervises nurse practitioners or clinical nurse specialists at designated sites.

Provides preventive, episodic, and acute/chronic care, including physical, functional, and mental health assessments.

Orders tests, interprets results, manages medications, and coordinates referrals.

Educates patients and families on health maintenance and illness management.

Travel locally using personal vehicle is required.

Requirements: Doctorate in Medicine, MI license, DEA registration, Board Certification or eligibility in Family Medicine or Internal Medicine, and at least 1 year of primary care experience with at-risk or elderly populations.

Schedule: Monday to Friday, 8:00 AM – 4:30 PM, with after-hours and weekend on-call rotations managed remotely.
