Seeking a full-time Physician passionate about team collaboration and community-based care to support elderly and adults with disabilities in Mt. Pleasant or Houghton Lake.
Ideal candidates are BE/BC in Family Medicine, Internal Medicine, Geriatrics, or Hospice, with at least 1 year of primary care experience with at-risk populations.
- Participate in developing and monitoring individualized care plans as part of an interdisciplinary team.
- Supervise nurse practitioners or clinical nurse specialists at designated sites.
- Provide preventive, episodic, and chronic care, including physical, functional, and mental assessments.
- Order tests, interpret results, and manage acute and chronic conditions, including medication regulation.
- Educate members and families on health maintenance and disease management.
- Travel locally using personal vehicle; valid driver’s license and insurance required.
Requirements include a Doctorate in Medicine, DEA registration, NPI, CPR & AED, MI driver’s license, and board certification or eligibility. The schedule is Monday to Friday, with remote after-hours and weekend on-call rotation.
