Physician – Live Well Homecare

Seeking a full-time Physician passionate about team collaboration, community-based care, and serving elderly and disabled adults to promote independence.

Participate in interdisciplinary teams to develop and monitor personalized care plans, providing geriatric and medical expertise.

Supervise nurse practitioners and clinical staff, offering preventive, episodic, and chronic care, including physical, functional, and mental health assessments.

Evaluate and manage acute and chronic conditions, order tests, interpret results, and adjust treatments accordingly.

Educate members and families on health maintenance and illness management.

Requirements include a Doctorate in Medicine, MI license, DEA registration, Board Certification or eligibility in Family Medicine or Internal Medicine, with geriatrics certification preferred. Minimum one year of primary care experience with at-risk or elderly populations required.

Schedule: Monday to Friday, 8:00AM–4:30PM, with remote on-call rotations. Benefits include competitive pay, PTO, health plans, retirement, and more. In-person work at Mt. Pleasant or Houghton Lake locations.
