We are seeking a full-time Palliative Care Nurse Practitioner (NP) to join our compassionate hospice team in Rochelle, IL, and surrounding areas.
The ideal candidate will:
- Manage a caseload of palliative patients, providing diagnosis, treatment, medication management, assessments, and consultations.
- Travel to patients’ homes or care facilities to deliver personalized, dignified care.
- Develop and enhance our palliative care program, maintaining comprehensive knowledge in hospice and palliative practices.
Requirements include a valid APRN license, RN license, NPI number or ability to obtain one, and willingness to collaborate with the Medical Director. Preferred experience is 1-2 years in nursing. A driver’s license, insurance, and vehicle are necessary.
