We are seeking a Nurse Practitioner or Physician Assistant to join our interventional pain management team across multiple locations, including outpatient clinics, a physical therapy office, and a surgery center.

Responsibilities include:
Conduct comprehensive assessments, diagnoses, and care plans
Order, interpret, and follow up on diagnostic studies
Document patient care accurately
Provide care per accepted standards and collaborate with team members
Monitor and refer patients to specialists as needed
Qualifications:
Current applicable state license and national certification
DEA license or ability to obtain one
Basic Life Support certification
This full-time role offers a salary between $125,000 and $150,000 annually. Candidates must be willing to relocate to Sutherlin, OR before starting. Work is in person, with a focus on outpatient care.
