We are seeking a dedicated Palliative Care Nurse Practitioner (NP) for a full-time role serving Rochelle, IL, and surrounding areas including Southern Rockford, DeKalb, and Sterling.
The NP will develop and manage our palliative care program, providing diagnosis, treatment, medication management, assessments, and consultations to patients in their homes or facilities. Responsibilities include traveling to patient locations to ensure comfort and dignity during end-of-life care, supporting patients and families through compassionate service. The ideal candidate holds a valid APRN and RN license, with experience in nursing preferred. They must have or be able to obtain an NPI number and potentially a Controlled Substance License and DEA number.
