Seeking a full-time Physician passionate about team collaboration and community-based care to serve elderly and adults with disabilities.
Provide comprehensive primary care, including preventive, episodic, and chronic illness management, within an interdisciplinary team.
Supervise nurse practitioners and clinical staff, develop care plans, and monitor patient health status.
Perform examinations, assessments, and interpret tests, exercising clinical judgment to manage conditions and medications.
Educate patients and families on health maintenance and illness management.
Travel locally using personal vehicle, with a schedule Monday to Friday, 8:00AM–4:30PM, with after-hours on-call.
Requirements include a Doctorate in Medicine, MI license, DEA registration, Board Certification or Eligibility in Family or Internal Medicine, and at least 1 year of primary care experience with at-risk or elderly populations.
