Seeking a full-time Physician dedicated to team collaboration and community-based care to support elderly and adults with disabilities in maintaining independence.
Requirements include a Doctorate in Medicine, board certification or eligibility in Family or Internal Medicine, Geriatrics preferred, MI license, DEA, NPI, CPR/AED, and at least 1 year of primary care experience with at-risk populations.
