Seeking a full-time Physician committed to team collaboration and community-based care to support elderly and adults with disabilities.
Responsibilities include developing care plans, supervising nurse practitioners, providing preventive, episodic, and chronic care, and performing physical, functional, and mental health assessments. Exercises clinical judgment, manages medications, and coordinates referrals, ensuring personalized, age-appropriate treatment. Educates patients and families on health maintenance and illness management.
Requirements include a Doctorate in Medicine, MI license, DEA registration, board certification or eligibility in Family Medicine, Internal Medicine, Geriatrics preferred, and at least 1 year of primary care experience with at-risk or elderly populations. Schedule is Monday to Friday, 8:00AM–4:30PM, with remote after-hours on-call. Benefits include competitive pay, generous PTO, health/dental/vision insurance, retirement plan, and flexible options.
