Palliative Care Nurse Practitioner (NP)

We are seeking a full-time Palliative Care Nurse Practitioner (NP) to join our compassionate team serving Rochelle, IL, and surrounding areas.

Manage a caseload of palliative patients, providing diagnosis, treatment, medication management, assessments, and consultations.

Travel to patients’ homes and facilities to deliver personalized, dignified care.

Develop and expand our palliative care program while maintaining expertise in hospice and palliative practices.

Collaborate with a multidisciplinary team dedicated to ensuring comfort and peace for patients and their families.

Requirements include a valid APRN license, RN license, and the ability to obtain an NPI number. Preferred candidates have 1-2 years of nursing experience, a controlled substance license, and a collaborative agreement with a Medical Director.
