Seeking a full-time Physician dedicated to team collaboration and community-based care for elderly and adults with disabilities.
Functions as part of an interdisciplinary team, developing care plans, monitoring health, and providing geriatric and medical expertise. Supervises nurse practitioners or clinical nurse specialists, offering preventive, episodic, and chronic care, including physical and mental health assessments. Evaluates, orders tests, interprets findings, manages medications, and coordinates referrals, ensuring personalized care aligned with patient goals. Educates patients and families on health maintenance and disease management. Requires Doctorate in Medicine, MI license, DEA, NPI, CPR/AED, with Board Certification in Internal Medicine or Family Practice; geriatrics certification preferred. Minimum of 1 year recent primary care experience with at-risk or elderly populations. Schedule: Monday-Friday, 8:00AM-4:30PM, with remote after-hours on-call rotation.
