Seeking a Speech Language Pathologist for our Home Care team, providing personalized evaluations and treatments in patients’ homes to improve communication, cognition, and swallowing skills.
Responsibilities include interpreting medical prescriptions, conducting assessments, developing evidence-based treatment plans, documenting progress, and educating patients and caregivers.
Collaborate with an interdisciplinary team, participate in care planning, and stay current with clinical best practices. Requirements include a Master’s in Speech-Language Pathology, CCC or CFY, NY licensure, and CPR certification. Prior experience with dysphagia and neurogenic disorders is preferred.
