Seeking a Speech Language Pathologist to provide in-home assessments and treatments focused on communication, cognition, and swallowing.
Responsibilities include interpreting medical prescriptions, conducting evaluations, developing personalized therapy plans, adjusting strategies based on progress, and documenting care in compliance with regulations.
Provide education to patients and caregivers, collaborate with interdisciplinary teams, and participate in team meetings to ensure coordinated care.
Qualifications include a Master’s degree in Speech-Language Pathology, CCC or CFY status, valid state licensure, and CPR certification. Experience with dysphagia and neurogenic disorders is preferred.
