APRN/PA – Post-Acute and Long-Term Care – Advanced Practitioner

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently to manage patient care in long-term care facilities.

They aim to

    Responsibilities include conducting exams, diagnosing, treating illnesses, developing care plans, providing preventive services, and collaborating with facility staff and external providers.
    They ensure compliance, maintain accurate documentation, participate in quality initiatives, and promote health and safety.

    The AP must work independently, demonstrate strong clinical judgment, communicate effectively, and be proficient with EHR and telemedicine tools.
    Qualifications include a relevant degree, at least 2 years of experience (long-term care preferred), and board certification.
