Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluations and treatments in patients’ homes, enhancing communication, cognition, and swallowing skills.
Responsibilities include interpreting medical prescriptions, conducting assessments, developing and adjusting evidence-based treatment plans, documenting progress, and educating patients and caregivers.
Collaborate with interdisciplinary teams, participate in care planning, and maintain licensure through ongoing education.
Qualifications: Master’s degree in Speech-Language Pathology, CCC or CFY, NY State license, and CPR certification. Preferred experience with dysphagia and neurogenic disorders.
