APRN/PA – Post-Acute and Long-Term Care – Advanced Practitioner

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA who independently manages patient care in PALTC settings, collaborating with facility staff to improve outcomes, enhance patient and family experiences, reduce hospitalizations, and lower costs.

Responsibilities include conducting exams, diagnosing and treating illnesses, developing care plans, providing preventive services, and managing chronic conditions. The AP works autonomously within scope, making clinical decisions and serving as the primary provider, while building collaborative relationships with leadership and interdisciplinary teams.

Key duties involve ensuring compliance, maintaining accurate documentation, participating in quality initiatives, and overseeing health and safety protocols. Strong communication, clinical decision-making, and technology skills are essential, along with physical ability to respond quickly in emergencies.

Qualifications include a relevant degree (BS-PAS/MPAS/DMSc/MSN-NP/DNP), at least 2 years of experience (preferably in long-term care), and board certification (AANP/ANCC/PA). The role is in-person at statewide facilities, offering comprehensive benefits.
