Physician

Seeking a full-time Physician passionate about team collaboration, community-based care, and serving elderly and disabled individuals to promote independence.

  • Functions as part of an interdisciplinary team, developing care plans, providing medical leadership, and managing acute and chronic conditions.
  • Performs physical, functional, and mental health assessments; orders and interprets tests; and adjusts treatments accordingly.
  • Provides preventive care, health education, and arranges referrals, ensuring comprehensive patient management.
  • Supervises nurse practitioners or clinical nurse specialists at designated sites.
  • Requires MD/DO degree, MI license, DEA registration, Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics, with at least 1 year of primary care experience with elderly or at-risk populations.
  • Schedule is Monday to Friday, 8:00AM–4:30PM, with remote after-hours on-call rotation. Benefits include competitive pay, PTO, health coverage, retirement plan, and flexible benefits.
