Seeking a full-time Physician passionate about team collaboration, community-based care, and serving elderly and disabled individuals to promote independence.
Functions as part of an interdisciplinary team, developing care plans, providing medical leadership, and managing acute and chronic conditions. Performs physical, functional, and mental health assessments; orders and interprets tests; and adjusts treatments accordingly. Provides preventive care, health education, and arranges referrals, ensuring comprehensive patient management. Supervises nurse practitioners or clinical nurse specialists at designated sites. Requires MD/DO degree, MI license, DEA registration, Board Certification or eligibility in Family Medicine, Internal Medicine, or Geriatrics, with at least 1 year of primary care experience with elderly or at-risk populations. Schedule is Monday to Friday, 8:00AM–4:30PM, with remote after-hours on-call rotation. Benefits include competitive pay, PTO, health coverage, retirement plan, and flexible benefits.
