Physician – Live Well Homecare

We are hiring a full-time Physician to join a mission-driven, community-focused organization dedicated to helping elderly and disabled individuals maintain independence.

Function as a team member in developing care plans, providing geriatric and medical expertise, and guiding treatment decisions.

Supervise nurse practitioners and conduct preventive, episodic, and chronic care, including physical, functional, and mental health assessments.

Evaluate members, interpret tests, manage acute and chronic conditions, and adjust medications as needed.

Educate patients and families on health maintenance and illness management.

Travel locally using personal vehicle; scheduled weekdays, with after-hours/on-call rotation manageable remotely.

Requirements include a Doctorate in Medicine, MI license, DEA, NPI, CPR/AED, Board certification or eligibility in Family or Internal Medicine, with geriatrics certification preferred. Minimum one year primary care experience with at-risk or elderly populations is essential.
