Responsibilities:
Evaluate, diagnose, and develop treatment plans for speech, language, cognitive-communication, and swallowing disorders.
Provide evidence-based therapy aligned with physician orders and facility policies.
Collaborate with an interdisciplinary team to ensure comprehensive patient care.
Educate patients, families, and caregivers on treatment goals and home programs.
Maintain accurate clinical documentation and participate in care planning meetings.
Qualifications:
Master’s Degree in Speech-Language Pathology from an accredited program.
Current state licensure or eligibility, and CCC-SLP from ASHA (CFY candidates welcome).
Preferred but not required: experience in a Skilled Nursing Facility.
Strong interpersonal and communication skills.
