Utilize nursing skills to develop and implement personalized care plans, ensuring regulatory compliance and high-quality care. Assist in staff orientation, supervision, and daily operations within assigned neighborhoods. Conduct assessments, manage care plans, and support interdisciplinary treatment interventions. Provide mentoring, develop Performance Improvement Plans, and promote person-centered care. Ensure accurate documentation, communication with providers, and timely reporting of events. Maintain regulatory compliance and support continuous quality improvement. Requirements include active PA RN license, BSN, CPR/BLS, wound care and infusion certifications preferred, with 3+ years long-term care and 2+ years acute care experience.
