Nurse Practitioner – Pulmonary – SNF / LTC Rehab center

We are seeking an experienced APRN in pulmonary care for a part-time, in-person role in skilled nursing and long-term rehabilitation settings.

Provide comprehensive pulmonary assessments, manage acute and chronic respiratory conditions, and perform procedures such as ventilator and trach care.

Collaborate with interdisciplinary teams, educate patients and staff, and participate in initiatives to reduce hospital readmissions.

Manage respiratory therapies, interpret diagnostics, and ensure compliance with regulations.

**Qualifications:** Active APRN license, Master’s Degree in Nursing, minimum 3 years of nursing experience, and experience in long-term care preferred.
