We are seeking an experienced APRN in pulmonary care for a part-time, in-person role in skilled nursing and long-term rehabilitation settings.
Provide comprehensive pulmonary and acute care, including assessments, diagnoses, and management of respiratory conditions such as COPD, asthma, and pneumonia.
Manage ventilator therapies, tracheostomy care, and coordinate weaning plans with the care team.
Perform admissions, discharges, and monitor treatment responses to prevent decompensation.
Collaborate with physicians, respiratory therapists, and staff, participate in care planning, and ensure documentation compliance.
Educate patients and families on respiratory care and protocols.
Qualifications: Active APRN license, NPI, BLS/ACLS, experience in tracheostomy and post-acute care preferred, EMR proficiency, and ability to work independently.
