We are seeking an experienced APRN in pulmonary care for in-person, part-time, contract work in skilled nursing and long-term rehab settings.
Provide comprehensive pulmonary and acute care, including admissions, assessments, and management of conditions like COPD, asthma, and respiratory failure. Manage noninvasive ventilation, tracheostomy care, and coordinate weaning plans with the team. Prescribe medications, interpret diagnostics, and monitor treatment responses to prevent decompensation. Collaborate with physicians, therapists, and staff, participate in rounds, and educate patients and families. Complete documentation, ensure compliance, and participate in initiatives to reduce hospital readmissions. Qualifications: Active APRN license, NPI, BLS/ACLS, experience in tracheostomy and post-acute care preferred, EMR proficiency, and strong clinical skills.
