Speech Language Pathologist (Home Health) (Part time/PRN)

We are seeking a licensed Speech Language Pathologist (SLP) for a Part-time/PRN role to assess, diagnose, and treat communication and swallowing disorders in patients’ homes.

Work with patients experiencing issues after stroke, brain injury, illness, surgery, or dementia.

Conduct initial assessments and develop individualized treatment plans.

Collaborate with the care team to ensure seamless patient care.

Record notes and progress in the health information system.

Requirements include current SLP licensure and a valid driver’s license. The role involves traveling to patients’ residences.
