Nurse Practitioner – Pulmonary – SNF / Long-Term Care

We are seeking an experienced APRN in pulmonary care for a part-time, in-person role focused on managing complex respiratory conditions in skilled nursing and long-term rehab settings.

Provide pulmonary and acute care, including admissions, evaluations, and discharge planning.

Manage respiratory conditions such as COPD, asthma, pneumonia, and ventilator/tracheostomy care.

Perform assessments, interpret diagnostics, and prescribe medications per regulations.

Collaborate with interdisciplinary teams and educate patients and families.

Participate in efforts to reduce hospital readmissions and ensure regulatory compliance.

Requirements include an active APRN license, experience in tracheostomy care, BLS/ACLS certifications, and ability to work independently two days weekly, including weekends. Compensation ranges from $60,000 to $90,000 annually, with flexible scheduling, ongoing education, and AI-assisted documentation support.
