Social Worker PRN

Join a dedicated non-profit organization serving Utah communities for over 30 years as a PRN Social Worker in Home Health & Hospice in Moab & Grand County.

This part-time role offers starting pay at $40/hr (up to $55/hr DOE & licensure), with travel stipends of up to $125/day. Responsibilities include conducting psychosocial assessments, providing crisis support, grief counseling, care planning, and resource referrals.

Requires MSW and active Utah licensure (LCSW preferred).

Experience in hospice or medical social work preferred.

Must have reliable transportation, pass background checks, and be available for weekly team meetings.
