PM&R Physician – Live Well Homecare

We are seeking a BC/BE PM&R Physician for full-time or part-time work with benefits in a growing, physician-owned group.

Provide medical oversight and care across subacute, long-term, assisted, and independent living settings.

Maintain timely, accurate documentation and participate in quality, utilization, and peer review programs.

Collaborate with staff and administration to support strategic and operational goals.

Participate in medical staff duties, meetings, and safety standards, ensuring compliance with regulations.

Work independently with good judgment, manual dexterity, and stamina; may require travel between facilities.

Qualifications include medical degree, licensure, and board certification or eligibility, with experience preferred in post-acute or hospitalist settings.
