We are seeking a Licensed Addiction Counselor (LAC) to join our Community Health Center, providing outpatient primary care and behavioral health services.
Summary: This role integrates Substance Use Services into the primary care setting, supporting development and implementation of treatment programs, care coordination, and direct services for patients.
Responsibilities include:
- Collaborating with medical providers to identify and refer behavioral health needs
- Conducting Substance Use Assessments
- Developing treatment plans and coordinating care
- Serving as a resource on Substance Use issues
- Providing care coordination and oversight for Medication Assisted Treatment (MAT) and mandated treatments
- Offering individual, group, and family therapy in line with evidence-based practices
- Maintaining accurate documentation and reports
- Participating in meetings and training
Qualifications:
- Licensed Addiction Counselor (LAC) in Montana
- Minimum 2 years’ clinical assessment and treatment experience preferred
- Ability to work in Hamilton, MT, with relocation required prior to start
