We are seeking a Licensed Addiction Counselor (LAC) to join our Community Health Center, providing outpatient primary care and behavioral health services.
Summary: The LAC will integrate Substance Use Services into the primary care setting, assisting with development, implementation, and coordination of care. They will offer assessment, treatment planning, and support for patients with substance use issues, including those in Medication Assisted Treatment (MAT).
Essential Duties:
Collaborate with medical providers to identify and refer behavioral health needs
Conduct Substance Use Assessments
Develop and coordinate integrated treatment plans
Consult with agency providers on Substance Use issues
Provide care coordination and oversight for MAT and mandated treatments
Deliver individual, group, and family recovery support based on evidence-based practices
Maintain accurate clinical documentation and reports
Participate in meetings and training
Requirements:
Licensed Addiction Counselor (LAC) in Montana
Minimum 2 years’ clinical assessment and treatment experience preferred
Ability to work in person in Hamilton, MT and relocate if needed
