Seeking an Advanced Practice Provider to deliver comprehensive outpatient primary care at a Family Care Clinic in Prineville, OR.
Provide patient evaluation, diagnosis, treatment, and documentation across all ages under supervision. Coordinate multidisciplinary services within a Medical Home model to optimize outcomes and cost-efficiency. Develop and monitor treatment plans integrating behavioral health, pharmacy, therapy, nutrition, or surgery, and refer to specialists as needed. Work at least 40 hours weekly with scheduled patient contact; participate in shared call rotations including evenings, weekends, and holidays. Collaborate with a team supporting evidence-based, personalized care, and ensure compliance with policies, laws, and quality standards. Maintain current licensure, certifications, and participate in ongoing education. Qualifications include a Master’s in Physician Assistant Studies or Nursing, valid licensure, and experience in outpatient/family medicine. Compensation ranges from $154,568 to $165,351 annually, with benefits including health insurance, retirement plans, and paid time off. Relocation to Prineville, OR, is required.
